With this ZonMw grant of €37.420 Marike van der Schaaf and her colleagues will set up an inter-professional network with the aim to realize a good transition from the hospital to the first line. In this network, specialized healthcare professionals and social professionals will provide person-centered support to people after a stay at ICU. The project builds on previous studies within the REACH research line (Rehabilitation After Critical Illness and Hospital discharge) focused on the rehabilitation of patients with Post Intensive Care Syndrome (PICS). The professionals in the network are experts in treating and supporting people with PICS and work together. The activities of the network are aimed at improving the perceived health, increasing the control of people with PICS and promoting their social participation. The project starts in October and will last for one year. Marike van der Schaaf is project leader and teacher researcher Mel Major will carry out the project.
